Original Coal Mining and Reclamation Applications and Permit Application Forms, applications for adjacent areas and IBR (Application for Incidental Boundary Revision) shall be submitted to:

Application for Coal Mining Permit Renewal, ARP (Application to Revise a Coal Mining Permit), and Application to Transfer a Coal Mining Permit or Coal Mining Permit Application shall be submitted to:
