Facility Registration Annual Reporting Forms

These forms are to be used by registered facilities for calendar year 2008 data.
NOTE: Data may be entered directly into this PDF form and printed, it may not be sent electronically to the Division of Water.  Users must print the completed form and mail it to the Division of Water at the address on the form.
